The 499/599 Report of an Investigation application forms are available at the 499/599 Information Desk, which is at the Management Student Centre, and are also available for you to print out below.  It should be noted the 499/599 Report of an Investigation application is a guide and additional pages may be attached to the proposal if there is insufficient space.

499/599 Application form Submitted - What Happens Next?

   o     499/599 Student Sponsor Contract

Sponsors are expected to cover all costs of the 499/599 project. The following is a sample of a contract to be used by students when confirming the payment of expenses by the sponsor for a 499/599 project.

In the case that a 499/599 student would like to bear the costs of a 499 project, a contract is not necessary. However, to ensure that sponsors agree and fulfill the obligations to cover expenses of a 499 project, a contract is highly recommended.

Sponsors do not have to reimburse any expenses incurred by 499/599 projects that students may claim for, unless there is a legal document that is binding.

Remuneration is not compulsory to students as the 499/599 is a requirement of the Bachelor of Management Studies. Students or sponsors may wish to negotiate remuneration options before a project is agreed upon and accepted by both parties. In this case, remuneration MUST also be written and signed by both parties to ensure a legal document is binding.

It is the responsibility of students to negotiate contracts with sponsors. Neither the University of Waikato nor any staff member accepts responsibility or liability for contracts formed between a student and a sponsor.

A 499/599 Report of an Investigation disclaimer form should be bound into the front of ALL 499/599 reports (after your cover page). The form is a "disclaimer from liability" and has been provided to protect students and the University of Waikato from professional indemnity claims. Professional indemnity is when a client (sponsor) or third party has suffered loss as a result of advice given by a professional (supposedly the student) and seeks legal action and compensation. The work of students is not considered to be the work of professionals, unless privately arranged between the sponsor and the student. However, a contract of this nature should specify that the University of Waikato accepts no responsibility for the advice given by students. ALL 499 students MUST take DUE CARE when making recommendations or giving any advice, written or spoken, to sponsors and potential users of the 499 report

o       Final report form

A final report form, student consent form and sponsor consent form should accompany all reports. Final report forms ensure that the 399/499/599 office has the correct details of a student's project on file.

Please note that 499/599 FINAL REPORTS ARE NOT TO BE HANDED INTO SUPERVISORS, but only to the Main Reception, Management Student Centre.

The 399/499/599 office takes no responsibility for Report of Investigations unless a final report form is presented. Therefore students should seek a signed, dated and stamped copy when they hand in their report to the Main Reception, Management Student Centre. 

Students should make sure that they keep both a hard and soft copy of their report.  Reports handed in for grading ARE NOT RETURNED.

If necessary a one-month extension may be applied for and will be granted with an automatic reduction of a letter grade which may be imposed in such cases (e.g. a B+ becomes a B). (Note: If the student is in receipt of a student allowance this allowance may not cover the additional one-month period). Where no approval has been sought for an extension, and the 26 week period comes to an end, the student will be automatically awarded an IC grade. They may be allowed to re-enrol in the paper but would be required to research a different topic.

Where a supervisor or sponsor availability has influenced the late submission of the 499/599 report, supervisors should allow an appropriate time extension without a grade reduction. Where serious disruption has occurred for medical or similar reasons, students may make a written request to their supervisor or DC for special consideration. This request MUST be made at the time the disruption is apparent, and reasonable allowance will be made subject to the approval of the supervisor or DC. Approval should be sought in the first instance from the supervisor/DC, who will then if necessary refer the matter to the Student Services Manager at the MSC. Supervisors and students should fill in and sign an Extension Form and return it to the 499 Office, so they have the correct final report due date on the database, (an extension form is in the student folder). Once the Extension Application is signed by the supervisor and student, it should be handed in to the Main Reception, Management Student Centre, who will scan it and pass it on to the 499 Office.  It is important that this procedure is followed and the 499 Office is given the original copy of the Extension Application.

The 499 Office gets requests from Studylink and the Immigration Department for copies of students' Extension Applications as these are required for the purpose of loans and allowances or student visas.
